The problem is that some institutions fail to do a good job of screening up front. As mentioned, UofS and UofA are not having problems. Neither are 8 of the 11 programs in the OUA. IIRC only Laurier, Queen's and York have had any eligibility problems in the past decade.
As someone who works with varsity team athletes on their academic eligibility, I would like to state that Queen's has not had any issues eligibility-wise since week 2 of 2014. We've also had systems instilled where the study hall mentors, their supervisors and varsity coaches all be contacted with regular interval with A&R in regards to academic eligibility for not just varsity team athletes but also varsity club athletes as well.
